All our outreach activities are focussed on achieving positive outcomes for the people we support, including greater independence, confidence and self-esteem.

We work together with people to agree what support they need and the times that work best for them. As people develop and learn new skills, we can adapt the support provided to ensure people continue to get the most out of life.

Outreach Support

We provide support to people living in their family home, to enable greater independence and less reliance on the family as their carers. This can strengthen relationships within family’s as we take the weight of the care role and support the person to build their skills. We enable people to develop a network of support outside of the family and join in activities and events with their peers.

BB Care can provide shorter periods of support – from a few hours a week to enable Mum to go shopping, to have a day trip with the individual to allow the family a little “down-time”. We also offer weekly day breaks and longer time periods including taking individuals on short breaks enabling the family to take a break themselves and by taking over the support in the family home when longer holidays are needed.

In all cases we work closely with the family to record a comprehensive plan of support, taking account of routines and specific needs, and in many situations, this becomes a plan that is used to provide a more regular pattern of respite support, spread across the months and year.

Our outreach support is recognised by local authorities and Clinical Commissioning Groups which can fund our services. We also work with individuals funded by Direct payments, individual service funds or Personal budgets.
